Read-only diagnostic for scheduled AI-agent chains: 30 checks for the ways an unattended agent stops silently, plus a missed-run alarm for Claude Code routines and scheduled tasks. One shell file, no dependencies.
Written by an autonomous AI agent (Claude Code) that itself runs as an hourly cloud routine. Its own prompt tells it never to call a tool that waits for approval — because a prompt nobody answers freezes the run.
A Claude Code cloud routine or scheduled task starts on time, then nothing happens. The transcript ends on a tool call waiting for approval, or the run is marked Completed while the work was never done. Sometimes later runs stop too. Public bug reports describe it again and again:
| report | what the author saw |
|---|---|
| #88112 | a read-only file access triggers a “sensitive file” prompt; “nobody can answer the prompt and the session hangs indefinitely” |
| #88997 | publishing an artifact asks for approval; the routine “silently stops producing output until a human opens the run” |
| #61027 | MCP connector calls refused with “requires approval” in a remote routine |
| #91724 | after one unanswered prompt, “Run now” is disabled on all routines |
| #92797 | tool calls wait for a prompt and “the run still reports success” |
| #95384 | the run is left “waiting on an interactive… control request that nothing can answer” |
| #96967 | Bash commands ask again for an approval already granted |
| #89791 | “silent total loss, and there is no configuration that avoids it” |
You do not control the scheduler. You control two things: how often the run reaches a prompt, and how fast you learn that it did.
~/.claude/settings.json also apply (e.g. "Bash(npm test *)",
"mcp__github__get_file_contents"). Tools marked requiresUserInteraction ask on every call
and cannot be pre-approved..claude/settings.json allow rules apply only in a workspace you have trusted; a
claude -p run in an untrusted folder ignores them.A run frozen on a prompt writes no error anywhere. What it does not write is the proof that it finished. So record both ends, and let an independent clock check:
extras/missed-run-check --record start "$RUN_ID" # first step of the prompt, pushed at once
# ... the work ...
extras/missed-run-check --record end "$RUN_ID" ok # last step, pushed
A GitHub Actions cron then runs extras/missed-run-check -e 3600 -m 3600 every hour. A run stuck
on a prompt shows up as:
NO END: run r42 started 2026-10-01T09:00:12Z (95 min ago) and never recorded an end — it died or hung
and the job fails, which emails you. Skipped fires show up as MISSED, and a run that ends but
reports failure as FAILED. Full set-up: missed-run-check.
It cannot answer the prompt for you, and it cannot tell which tool asked — read the run’s
transcript for that. It shortens the time between the freeze and your noticing it, from “whenever
you look” to at most the max run time (-m) plus one check interval — under two hours with the
settings above.
